By participating in the Quiet Bodies/Kala Fletcher’s virtual workouts/classes and or sessions I acknowledge that the workout Pilates being conducted involves strength and flexibility exercises which can be potentially hazardous activities.

I also acknowledge that in the instance of online workouts, all or part of the session/class/workout will be held off-premises of Quiet Bodies/Kala Fletcher. I understand that these premises, including the equipment on these premises, are maintained by a third party. Quiet Bodies/Kala Fletcher has not undertaken any inspection of the premises or the equipment on the premises and makes no representation as to the condition of said premises or equipment. I am voluntarily agreeing to participate in the workout/class/session and I hereby agree to expressly assume and accept any and all risks of injury or physical harm. I acknowledge and represent that I am physically sound, and I do not suffer from any illness, impairment, disease or other condition that would prevent me from participating in the workout/class/session.

In consideration of being allowed to participate in the workout/class/session, I do hereby knowingly and voluntarily, on behalf of myself and my heirs and assigns, forever waive, release, discharge and hold harmless Quiet Bodies/Kala Fletcher from any and all liability, damages, losses, suits, demands, causes of action (including, without limitation, negligence) or other claims of any nature whatsoever, including, without limitation, any losses for property damage or personal injury, arising out of or relating in any way to my participation in the workout/class/session and its related programs and activities or my use of any equipment or machinery in connection with the workout/class/session.

I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident, and/or illness during the workout/class/session. I represent and acknowledge that I have read and understand this Liability Waiver and Release. The invalidity, in whole or in part, of any portion of the above paragraphs, will not affect the remainder of this form. My voluntary execution of this form evidences my agreement to the terms, provisions, waivers and releases as set forth above.

Private/Duet and Semi-Private Sessions are all subject to a 24-hour cancellation policy. In the event a cancellation is made with less than 24-hours notice, the session will be deducted from the account as if the session took place.

Group Classes are all subject to a 12-hour cancellation policy. In the event a cancellation is made with less than 12-hours notice, the session will be deducted from the account as if the session took place.
